​​1.      Project managers for MOH and SGH initiated Value Driven Care (VDC) conditions, ensuring the progress of VDC end-to-end.

2.      Support and coordinate relevant value-based initiatives such as the following:

3.      Drive and implement hospital-wide campaigns in line with promoting value based healthcare e.g. Choosing Wisely, Patient Blood Management

4.      Collaborate with relevant stakeholders to write academic papers showcasing the improvement efforts made by the Office of Value-Based Healthcare

5.      Collaborate with external local and international stakeholders involved in value based healthcare to learn from and promote best-in-class practices.

6.      Provide necessary secretariat support to the relevant meetings.

Key Accountabilities:

-         Literature review of the VDC condition, commonly used stratification (if present), indicators and international benchmarks

-         Immersion programme on the ground to better understand the patient journey and glean insights on existing pain points

-         Collaborate with SingHealth Office of Value Driven Care (OVDC), clinicians, nurses, and allied health professionals to define and finalise population, clinical quality indicators and/or patient reported outcome measures for SGH initiated VDC conditions.

-         Background analysis of baseline population characteristics to evaluate feasibility and clarify existing performance.

- Where needed, to discuss with external stakeholders to align on population and indicators

-        Set up data extraction algorithm according to MOH and SGH VDC Technical Manual.

-         Create / revise Work Instruction (WI) to analyse the VDC performance based on the definition, inclusion, exclusion criteria of the population / indicator.

-         Quarterly analysis and reporting of CQIs for the VDC conditions.

o   Where conditions are not automated into dashboards, quarterly VDC reports are created and results reviewed before sending to clinicians

o   Where there are dashboards available, dashboard results are reviewed and sent to clinicians.

-         Conduct case review of population and cases that do not pass the indicator to identify areas for improvement or for data validation purposes. Analyse the data and perform deep dive data analysis, if required.

-         Discuss with clinicians regarding areas of improvement and initiate QI projects if applicable.

-         Update VDC conditions’ performance into VDC Masterfile and BSC report.

-         Work closely with clinician leads and relevant stakeholders for case review inputs, data analysis, and data validation.

-         Liaise with SingHealth OVDC to provide feedback to MOH regarding MOH Technical Manual

- Initiate and conduct handover of VDC to clinical departments when conditions are stable and no more QI projects are planned

-         Liaise with relevant stakeholders to develop or revise electronic CCP, ordersets or other relevant changes in electronic medical records (EMR) for the relevant VDC development and improvement.

-         Conduct literature review and revise pathway document as and when required with team members

-         Liaise with Medical Informatics (MI) and Nursing Informatics (NI) to optimise electronic pathway algorithm and documentation

-         Collaborate with Division Clinician Informaticians, stakeholders and IT team to prioritise of the changes to be made in EMRLiaise with relevant executives and stakeholders for the monitoring and reporting of the outcome measures for changes made in EMR

--         Collaborate with relevant stakeholders (e.g. clinicians, Medical Informatics, Health Services Research Unit) to develop and implement the PROMS for VDC conditionsTimely reporting of outcome of PROMS implementation to identify areas for improvement for the VDC conditions

--         Identify hospital level campaigns that can advance the practice of value based healthcare

-         Craft out roadmap that is informed by Implementation Science and present to senior doctors and management for alignment

-         Implement campaigns via pilots with the view towards sustainability and expansion

o   Run baseline data to identify and finalise areas with clinicians, allied health and administrators which would benefit most in line with the campaign

o   Create pilot QI projects or implementation plan and track data to look for any changes post implementation

o   When results are achieved and stable, to expand to a wider population and/or hand back to clinical departments

-         Run hospital wide publicity programmes to promote awareness and address barriers to adoption of campaign

o   Create and publish multi-channel publicity ranging from posters, roadshows to other outreach events Create interactive outreach events to identify ground sentiment, barriers, and to track for changes in these during the course of the campaign

--         Assist to automate the process of extracting, handling and processing data.

-         Perform data validation on the automated data or dashboards.

-         Assist to ensure optimal presentation of information and data through the use of available data visualisation tools.Refresh VDC dashboard quarterly or as needed for presentation or monitoring purposes

-    Conduct orientation and briefing as needed
- Develop training materials

- -         Engage stakeholders (clinicians and other healthcare professionals) on data-driven improvement activities.

-         Collaborate with other SingHealth institutions to establish and implement cluster value improvement plans.

-         Participate and contribute to projects related to quality improvement initiatives Document the progress of the QI projects and assist with submission of posters/abstracts of the QI projects

--         Collaborate and assist with academic related write-up for the VDC conditions

- Submit the academic paper to the relevant platforms

--         Provide secretariat support for the relevant meetings including meeting arrangement and writing of minutes of meeting.

Perform ad-hoc duties as and when assigned by the department


Qualifications

-          Nursing, Biomedical Science, Biological Science, Applied Science or any other degrees in healthcare.

-       Proficient in the use of Microsoft Office applications. Proficiency in the use of statistical and data analytic software is an added advantage.

-        Experience in quality, process improvement, data analytics, clinical audit, or patient safety would be an advantage

- Experience in the healthcare industry, medical informatics and informatics-related project management would be an advantage
